INSTRUCTIONS TO CLAIMANTS
Eligibility Criteria
To be eligible, you must meet all of the following criteria:
- You purchased a transit fare (monthly, annual, or ticket booklet);
- You used this fare to travel on the following lines:
- Deux-Montagnes commuter train line;
- Mascouche commuter train line;
- You traveled during the period between November 1, 2017 and February 28, 2018.
The deadline for submitting a claim is October 20, 2026.

DECLARATION OF CONFIDENTIALITY
Your personal information is collected, used and retained by Class counsel and Proactio in accordance with applicable privacy laws and regulations:
- for the purposes of operating and administering the Quebec Class Action Settlement Action-Autonomie et al. c. Centre intégré de santé et de services sociaux de la Montérégie-Centre et al.;
- to assess and review the Claimant's eligibility under the Settlement Agreement; and
- are strictly private and confidential and will not be disclosed without the express written consent of the claimant, except as provided in the Settlement and Compensation Agreement
